CLEVELAND INSTITUTE OF ART EQUIPMENT LOAN AGREEMENT 

In exchange for loan and use of Cleveland Institute of Art departmental equipment, borrower agrees to the following CONDITIONS:

INSURANCE

The Borrower is responsible for the full cost of repair or replacement of any or all of the Equipment that is damaged, lost, confiscated, or stolen from the time Borrower assumes custody until it is returned . Borrower shall maintain adequate insurance coverage to cover loss or damage to the Equipment . A Certificate of Insurance may be required. If the Equipment is lost, stolen or damaged, Borrower must immediately notify the Cleveland Institute of Art departmental representative.

EQUIPMENT USE/ DISCLAIMER

The Borrower shall be responsible for the proper use and handling of loaned equipment. The Borrower accepts responsibility for conditions of the loan of equipment.

THE CLEVELAND INSTITUTE OF ART DISCLAIMS ALL REPRESENTATIONS AND WARRANTIES ASSOCIATED WITH THE EQUIPMENT AND ANY USE THEREOF, INCLUDING ANY WARRANTY OF USE FOR A PARTICULAR PURPOSE.

DELIVERY AND RETURN OF EQUIPMENT

The Borrower is exclusively responsible for equipment that is loaned and is liable for any and all damages or loss that occurs during the loan period, whether or not the equipment is in Borrower's possession during the loan period. 

Failure to return equipment by the return date and time inconveniences others. Late return of equipment results in Borrower assuming late fee charges, and may result in the loss of borrowing privileges. 

Equipment Checkout Return Policy

The majority of our items are due back within 24 hours of the start of your booking. You can see exactly when your items are due back under My Bookings in your dashboard.

Equipment Checkout Fine Policies

Late Fines - All equipment checkout items are subject to late fines if returned after the set return time upon picking up. All late returns within the first hour are assessed a flat $5.00 fine total regardless of the number of items. After the first hour each individual item accrues at $.25 per hour until returned. Once an item is overdue it cannot be renewed and must be returned.

Uncharged Battery Fine – Any camera returned without a FULLY charged battery will accrue one $10.00 fine per battery. 

Unattended Equipment Fine – Any item that is left unattended and is returned to checkout by anyone other than the person who’s name it is checked out to will accrue one $10.00 fine per item. Do not leave equipment unattended in studios, it will be repossessed and accrue a fine. In an emergency situation, a person may be dedicated to bring in any checked out items pending approval from Equipment Checkout Manager Only.  Contact Lauren Voss directly at 216 421 7944 or lvoss@cia.edu for emergency arrangements.


*All fines include a ban on renting equipment until fines have been payed.

If a student is late a total of THREE times in a semester that student will receive a ONE WEEK ban each subsequent late fine. This will reset at the start of the next semester provided all fines have been paid.

 

Equipment Checkout Renewal Policy

All items are available for a one time renewal provided no one else needs that item. If you want to renew something you must come in person to the checkout before that item is due. You may not book the item again online. Any subsequent bookings will not be honored.
Once an item is renewed it must be returned and is not eligible to be checked out again until 24 hours have passed.

*Renewals over the phone or via email will not be accepted except in emergency situations at the discretion of the Equipment Checkout Manager.

SiSo Software Limited are not responsible or liable in any way for any errors, damages or any other loss caused by your usage of this system. For the avoidance of doubt this includes any loss howsoever suffered including negligence or fraud except where such negligence causes death or personal injury.
